Addressing social determinants of health (SDOH) with Reema Health

Reema Health is a community-based care organization that provides one-on-one support and social needs navigation to State Health Plan members looking to improve their social, emotional or physical well-being. Reema’s services are available to all State Health Plan members, including those with Medicare as their primary coverage, at no member cost.

Social factors impact health as much as medical care. When members face barriers, such as transportation, housing or financial insecurity, they’re more likely to experience social isolation and loneliness—leading to poorer mental and physical health.

  • Limited transportation isolates members from loved ones.
  • Financial strain makes social connections harder to maintain.
  • Unsafe neighborhoods discourage engagement.

Over time, these barriers deepen isolation, increasing risks of depression, anxiety and chronic illness. By addressing SDOH, Reema helps members stay connected, engaged and supported—improving their overall well-being.

Outreach to members

Reema Health might reach out by mail, phone, text or in-person visits based on what works best for the member. Members can also self-enroll by visiting reemahealth.com/peba or calling Reema at 833.262.8875.

Reema is a trusted vendor of BlueCross BlueShield of South Carolina (BlueCross). Through its contract with BlueCross, PEBA has access to Reema. If the member is meeting their Reema community guide in person, the community guide will always have an official badge to show the member to confirm their identity. The member can also call BlueCross Customer Service at 803.736.1576 to confirm their identity.
